Please provide any details that are important for the client's treatment in the box below.  This includes primary language if other than English. Translation services are made available as needed for the family.

Please send documents to intake@masspediatricaba.com

- Original Diagnosis report with testings and scores

  (ex. ADOS, CARS, Vineland)

- Letter of medical necessity

- Recent physical 

- Front / back of all applicable insurance cards

* Client will remain on intake que until all documents are received. Once obtained client is added to waiting list

We're offering Parent Education sessions while waiting for direct services. Typically one hr per week via telehealth. Are you interested?
If a BCBA is available via telehealth for supervision would you be interested? (The BT would still come to your home for direct therapy every session)
